    As a result of this donation, I request my commemorative plaque reflecting that I will be a _____________________________ (List one of either PLATINUM, GOLD, SILVER, BRONZE or PATRON) sponsor for the sponsorship fund; (For PLATINUM sponsorship i need to donate $10,000.00 or more; for GOLD sponsorship I need to donate between $5,000.00 and $9,999.99; for SILVER sponsorship I need to donate between $2,500 to $4,999.99; for BRONZE sponsorship I need to donate between $1,000.00 and $2,499.99; for PATRON sponsorship i need to contribute between $500 to $999.99).

    In addition, GRAND DONATIONS of $100,000.00 in one lump sum payment, or in installment payments which are effectuated over a consecutive ten year period, allows a GRAND DONOR to name a scholarship in perpetuity, for as long as the organization exists. If you are interested in becoming a GRAND DONOR, please contact the Scholarship Fund President to arrange this scholarship.
